Patient portal

Access your patient chart, view test results, and message our staff and your doctor directly through our cloud based electronic medical record system.

For existing patients, ask your provider or our office staff how to set up an account to our online portal Elation Passport.  You may directly message your physicians through the portal, but keep in mind some messages may incur a charge.

You will not be billed for:
    Requesting a prescription refill
    Questions that lead to an appointment
    Questions related to a visit you had in the last seven days
    Questions about billing
    Messages initiated by your healthcare team

Here’s what you could get billed for:
    Requests for new medications
    Questions about new symptoms or problems
    Changes to a chronic condition
    Requests to fill out medical forms
    Requests for new referrals

Most insurance will cover the costs of the portal messages and patients will only be responsible for their co-pay or deductible.

At the Cardiovascular Medical Group, we manage out patient visits through the cloud based, fully encrypted and HIPAA compliant electronic health record (EHR) called Elation.

Signing up for Elation will allow you to

  • Securely send and receive messages directly from your doctor

  • Request and receive office notes and diagnostic and procedure reports

  • Access your medication and allergy information
